OK. Here're the facts:

-I have a free webspace with bravehost called http://chadlouwerse.bravehost.com.
-I created the pages with Frontpage.
-I uploaded them with the FTP applet bravenet provides.
-There IS a proper index.html
-If I type the URL of the site in my browser, my pages come up.

Here's the situation:

On the first website wizard page it gives you a list of sites that you have previously published, or the managing site (http://chadlouwerse.bravehost.com in my case). Underneath that is says "Last Published". In my case, it says never.

Here's the question:
If the wizard says that my site isn't published and I can see the site when I put the URL in my browser, is it really published or isn't it?

The bonus questions:
How do I publish pages when I use the visual/text editor? There's no happy "PUBLISH THIS NOW" button. How can I verify that they're published?

You are really working hard at making life unecessarily difficult.

"Publishing" is a term internal to the Wizard and involves the copying of files from the Wizards internal workspace to Bravenet's live servers. You didn't use the Wizard to build your site so ignore it, leave it entirely alone.

The visual editor directly updates the live pages, there is no "publishing" as your change is already live.
